Please download and print The Membership form, and bring the form with you next time you visit Buds2000.
Identification and proof of age (I.e. Portman group proof of age ID card, valid driving licence, birth certificate) is required before we can supply you with a free buds membership card so please bring it along with your application form. Strictly 18s and over may apply.
